For your initial visit at St. Anthony Health Care, you will need:

We have compiled the packet into PDF format. Be sure your computer is capable of displaying PDFs and you have a printer. Please FAX your completed packet to 941-249-3119, or drop it off at our office.
Request a Packet by Mail
If you would rather have your packet mailed to you, just contact us directly. We would be more than happy to send you the packet by mail at no charge.
What Next?
After completing the packet please visit our practice as soon as possible so that we may begin requesting your medical records. After your records have been collected you will receive a phone call informing you of our schedule for your Initial Evaluation.
